Construction and application of early phase Ⅰ cardiopulmonary rehabilitation program for patients with Type A aortic dissection

Objective To construct early phase Ⅰ cardiopulmonary rehabilitation program for patients with Type A aortic dissection and to evaluate its clinical effects. Methods From January 2021 to October 2022,type A aortic dissection patients of a tertiary hospital in Beijing was selected by the destination sampling. They were divided into a control group and an intervention group. The intervention group was given early phase Ⅰcardiopulmonary rehabilitation program for patients with type A aortic dissection and the control group was given routine care. The 6-minute walking experiment(6MWT),Borg subjective fatigue score,the first time out of bed,the incidence of hypoxemia and related complications,and the incidence of adverse events related to rehabilitation nursing were compared between the 2 groups. Results A total of 130 cases were selected,among which 20 cases were excluded because of surgical complications and self-withdrawal. A total of 110 cases were finally included,with 57 cases in the intervention group and 53 cases in the control group. The results of walking distance of 6MWT,Borg subjective fatigue score and incidence of hypoxemia had statistical significance between the 2 groups(P<0.05). The results of the time of the first time out of bed and the incidence of complications and the incidence of adverse events related to rehabilitation nursing had no statistical significance between the 2 groups(P>0.05). Conclusion Early phase Ⅰ cardiopulmonary rehabilitation program for patients with type A aortic dissection benefits the improvement of the motor function,the reduction of incidence of hypoxemia.

This research provides an overview of the perioperative nursing care for a patient with a massive renal cell carcinoma and Mayo Stage Ⅲ tumor thrombus who underwent a robot-assisted nephrectomy complicated by significant intraoperative bleeding and CO2 embolism. Key nursing considerations included multidisciplinary preoperative discussions,developing stepwise intraoperative emergency plans,establishing an integrated nursing workstation,and the optimization of patient monitoring. During the surgery,nursing efforts were focused on supporting the robot-assisted procedure,closely monitoring the patient’s condition,preventing intraoperative complications,initiating emergency plans as needed,actively managing major bleeding and CO2 embolism,and implementing structured positioning to minimize the risk of falls during the procedure. Postoperatively,the focus was on preventing secondary thrombus formation,managing active bleeding,and monitoring for liver and kidney ischemia-reperfusion injury. Through multidisciplinary interventions and meticulous nursing care,the patient was discharged after a 13-day postoperative recovery.

Construction and validation of a postoperative lung infection prevention care program for lung cancer patients

Objective To construct a postoperative lung infection preventive care program for lung cancer patients and carry out preliminary validation,so as to provide a reference basis that can be used in clinical practice. Methods Literature analysis was carried out to search domestic and international databases,guideline networks,and association websites,with a search timeframe of January 2011 to December 2021,to obtain relevant evidence,to construct a preliminary draft of a preventive care program for postoperative lung infections in patients with lung cancer,and to use the Delphi method of correspondence with experts from Guangdong Province,Henan Province,and Sichuan Province,September-December 2022,to determine the content of the program. Correspondence to determine the content of the program. In November-December 2023,15 thoracic surgery nurses and 15 lung cancer patients in a tertiary hospital in Shantou City were selected for the initial application of the program and evaluated the effectiveness of the program through the nurses’ clinical significance,applicability,feasibility,and patient satisfaction. Results A total of 21 experts completed 2 rounds of correspondence. 95.65% and 95.45% of questionnaires were returned in the 2 rounds of correspondence;the authority coefficients of the experts were 0.941 and 0.948;the Kendall’s harmony coefficients were 0.176 and 0.310;the coefficients of variation for each item in the 2nd round of correspondence were 0.04~0.25. The final draft of the program included 10 aspects and 37 measures,such as teamwork,health education,risk assessment,rehabilitation training,fluid management,oral manage-ment,airway management,nutritional management,pain management,line management,and other 10 areas with 37 measures. After the initial application of the program,100% of the nurses thought that the program was meaningful,87% of the nurses thought that the program was clinically applicable and feasible,and 100% of the patients indicated that they were satisfied with the program,of which 80% were very satisfied. Conclusion The constructed nursing program for preventing postoperative lung infection in lung cancer patients is scientific,relevant and feasible,and can provide references for clinical health care personnel to develop measures for preventing postoperative lung infection in lung cancer patients.

To sum up integrative nursing experience of a case with chronic refractory wound formation caused by drug extravasation. The essentials of integrative nursing are:structured nursing intervention of “assessment-management-treatment” based on the Triangle of Wound Assessment;determination of the timing for integrative nursing according to the theory of TCM sores and ulcers;implementation of copper board scraping method to promote circulation of qi and blood;use of Huo-long Comprehensive Moxibustion Therapy to promote muscle regeneration. With the help of the cooperation of the multidisciplinary specialist nursing team,the wound was completely healed after 59 days of integrated traditional Chinese and Western medicine nursing interventions.

Objective To explore the effect of Traditional Chinese Medicine’s five-element music therapy combined with individualized emotional counseling intervention in patients with autoimmune liver disease of liver depression and spleen deficiency type. Methods 74 patients with autoimmune liver disease of liver depression and spleen deficiency type who were admitted to the hepatology department of a tertiary hospital from January 2021 to February 2022 were selected as research subjects,and they were divided into an experimental group and a control group by a random number table with 37 cases in each group. The control group was given routine treatment and nursing care,as well as Traditional Chinese Medicine’s emotional nursing. The experimental group received Traditional Chinese Medicine’s five-element music therapy combined with individualized emotional counseling intervention on the basis of routine treatment and nursing care,with an intervention period of 4 weeks. The Generalized Anxiety Disorder Scale (GAD-7),Patient Health Questionnaire Depression Scale (PHQ-9) and Pittsburgh Sleep Quality Index(PSQI) were used to evaluate patients within 24 hours after admission and after 4 weeks of intervention. Results A total of 72 patients completed the study,with 36 in the experimental group and 36 in the control group. Before and after intervention,the scores of GAD-7,PHQ-9,and PSQI in the experimental group were compared,and the differences were statistically significant (P<0.001). After the intervention,the GAD-7 score in the experimental group was (7.89±3.15),which was lower than (10.14±4.41) in the control group. The PHQ-9 score was (12.33±4.08) in the experimental group,which was lower than (14.64±5.34) in the control group. The PSQI score was (12.39±3.06) points,which was lower than (14.22±3.37) in the control group,and the differences were statistically significant (P<0.05). Conclusion The application of Traditional Chinese Medicine’s five-element music therapy combined with individualized emotional guidance intervention can effectively improve the sleep quality and help alleviate negative emotions in patients with liver depression and spleen deficiency type of autoimmune liver disease.

Construction and preliminary application of a telemedicine-based home care program for hospice patients

Objective To construct a telemedicine-based home care program for hospice patients that will provide a reference for hospice telemedicine-based home care practices. Methods Using the Supportive Care Model as a theoretical guide,a preliminary telemedicine-based home care program for hospice patients was developed based on literature research combined with qualitative interviews. 2 rounds of expert consultation were conducted in March 2023 to solicit their opinions and revise the proposal to form the final draft. In April 2023,9 patients who were about to be discharged from the palliative care ward of a cancer hospital in Hunan Province,were selected for the preliminary application of the program by the convenience sampling method;the number of cases of adverse events and the Telemedicine Satisfaction Questionnaire were used for the effectiveness of the program. Results A total of 16 experts completed 2 rounds of consultation,and the coefficient of expert authority was 0.928. The effective questionnaire recovery rates of the 2 rounds of consultation were 94.12% and 100%;the expert opinion submission rates were 100% and 25.00%;the coefficients of variation of item scores were 0.05~0.18 and 0~0.16. The coefficients of Kendall harmony in 2 rounds were 0.093 and 0.210(P<0.001). The constructed final program included 4 dimensions of symptom management,daily life care,psycho-spiritual care and social information support,and a total of 17 modules such as pain management,dyspnea management,gastrointestinal symptom management and cough management,which covering 37 specific items. No adverse events occurred during the application and the score of care satisfaction was(60.22±7.41),which indicated a high level of satisfaction. Conclusion The program constructed in this study is scientific,relevant and feasible,and can provide guidance for healthcare professionals in conducting hospice telemedicine-based home care practices.

The development of a health-promoting lifestyle scale for patients with polycystic ovary syndrome and the test of its reliability and validity

Objective To develop a health-promoting lifestyle scale for women with polycystic ovary syndrome (PCOS),and to test its reliability and validity,and preliminarily apply it. Methods Based on the Pender health promotion model,the item pool of the scale was constructed through literature research,semi-structured interviews and group discussions. After 2 rounds of Delphi expert consultation and pre-survey,the initial scale was formed. From April to July 2022,316 patients with PCOS in the health management center,reproductive medicine center and endocrinology department of a tertiary hospital in Nanjing were selected for item analysis,exploratory factor analysis and reliability test,respectively. From August to October 2022,358 PCOS patients were selected for confirmatory factor analysis. From November 2022 to February 2013,294 PCOS patients were selected,and the scale was used to investigate the status of health-promoting lifestyle in PCOS patients. Results The health-promoting lifestyle scale for PCOS patients included 5 dimensions and 33 items. The total content validity index of the scale was 0.942,and the content validity index of each item was 0.810-1.000. 5 common factors were extracted by 2 exploratory factor analyses,and the cumulative variance contribution rate was 62.399%. Confirmatory factor analysis showed that the model fit was good. The Cronbach’s α coefficient of the total scale was 0.930;the split-half reliability was 0.842;the test-retest reliability was 0.888. The preliminary application results showed that the total score of health-promoting lifestyle in PCOS patients was(96.925±14.273),and the average score of items was (2.937±0.433),which was at a medium level. Conclusion The health-promoting lifestyle scale for PCOS patients has good reliability and validity,which can be used as a tool for medical staff to assess the level of health-promoting lifestyle of PCOS patients,and can help nurses to quickly identify the level and dimensions of health-promoting lifestyle of patients,so as to formulate individualized precise health management plans.

General well-being and its influencing factors in peer educators of breast cancer

Objective To investigate the general well-being of breast cancer peer educators and to analyze its influencing factors. Methods Convenience sampling method was used to select 210 breast cancer peer educators from 8 tertiary hospitals in China from January to February 2023. General information questionnaires,General Well-Being Scale,Cornor-Davidson Resilience Scale,and Social Support Rating Scale were used for investigation. Multiple linear regression analysis was used to explore the influencing factors of the general well-being in peer educators of breast cancer. Results A total of 206 breast cancer peer educators were investigated. The general well-being score was(86.70±14.08). The results of multiple linear regression analysis showed that psychological resilience,the utilization of social support and the current self-funded treatment of less than 1,000 yuan per month were the influencing factors of general well-being(P<0.001). Conclusion The general well-being of breast cancer peer educators is at a high level. Improving the social support system for breast cancer patients,selecting peer educators with strong psychological resilience,high utilization of social support and low self-funded treatment,and paying attention to the psychologic status of peer educators,providing timely and positive psychological interventions will help to raise the general well-being of breast cancer peer educators and ensure the quality and continuity of peer education.

Application of a management plan for microaspiration of oropharyngeal secretions in ICU patients with tracheal intubation

Objective To construct and apply a management plan for microaspiration of oropharyngeal secretions in ICU intubated patients. Methods Based on evidence summaries and expert consultation,a management plan for microaspiration of oropharyngeal secretions in ICU intubated patients was constructed,consisting of 19 items covering 7 aspects including identification of risk factors,position management,tube and cuff selection,cuff management,mechanical ventilation management,pain and sedation management,removal of oropharyngeal and subglottic secretions,and oral care. Convenience sampling was used to select 141 ICU intubated patients from a tertiary A comprehensive hospital in Suzhou from June,2022 to September,2023. Patients were divided into an experimental group(n=72) and a control group(n=69) according to the wards. The experimental group received the management plan for microaspiration of oropharyngeal secretions in ICU intubated patients. The control group received the nursing bundle for ventilator associated pneumonia(VAP). The incidence and time from intubation to microaspiration and VAP,duration of mechanical ventilation,ICU length of stay,and disease outcome were compared between the 2 groups. Results The incidence of microaspiration of oropharyngeal secretions,the duration of mechanical ventilation,time from intubation to microaspiration showed significant differences between the 2 groups(P<0.05). There were no significant differences in the incidence of ventilator associated pneumonia,ICU length of stay,and disease outcome between the 2 groups(P>0.05). The time from intubation to VAP in the experimental group was 7.5 days, and that in the control group was 3.8 days. Conclusion The application of the management plan for microaspiration of oropharyngeal secretions in ICU intubated patients is beneficial for reducing the incidence of microaspiration,delaying the time from intubation to microaspiration and VAP,and shortening the duration of mechanical ventilation.

The application of mindfulness-based intervention in nurses’ compassion fatigue:a scoping review

Objective To analyze and summarize the strategies to improve nurses’ compassion fatigue through reviewing the application research of mindfulness-based intervention,to provide the guidance for clinical medical staff to carry out mindfulness-based intervention and related research. Methods According to the research method of scoping review, a literature search was performed in the CNKI, Wanfang, CBM, PubMed, Embase, CINAHL, Web of Science,Scopus,ProQuest, the Cochrane Library. The retrieval time limit was from the establishment of the databases to June 15,2022. The included studies were summarized and analyzed. Results A total of 26 articles were included. The main elements of mindfulness-based interventions include curriculum theory introduction,mindfulness skills training,compassion training,stress coping guidance and mindfulness commitment practice. The main forms of mindfulness intervention are offline training,online training and mixed training. The frequency and length of training for the theoretical course of mindfulness intervention is mostly once a week,and 1.0~2.0 h per session;the frequency and length of skill training is mostly 2~4 times per week,and less than 30min per session. The outcome evaluation indicators include application effectiveness,feasibility,and application experience. Conclusion Mindfulness-based intervention has a variety of content elements and implementation strategies,and has positive application effects on nurses’ compassion fatigue. In the future,we need to continuously optimize the content and strategies of mindfulness intervention by combining the Internet and intelligent technologies,and conduct more high-quality randomized controlled studies to verify their long-term intervention effects.

Objective To explore the barriers and facilitators factors of adherence to complete decongestive therapy(CDT) in patients with breast cancer related lymphedema(BCRL). Methods Based on the Health Action Process Approach theory,from September 2021 to February 2022,12 patients with BCRL undergoing CDT were purposely selected for semi-structured in-depth interviews. Data were collected and analyzed using the Colaizzi seven-step analysis method. Results The extracted barriers of CDT included insufficient risk perception,negative outcome expectation,low self-efficacy,lack of behavioral intention,lack of action and coping plan. The facilitators included perceived benefits of treatment,perceived harm of edema,enhanced self-efficacy,motivation of treatment,and construction of a new normal life. Conclusion The adherence of CDT in patients with BCRL is affected by a series of barriers and facilitators. In the future,targeted intervention strategies can be formulated based on the theory of health action process approach to improve the treatment adherence.

Prevalence of sleep disorders in post-ICU patients: a systematic review

Objective To systematically evaluate the prevalence of sleep disorders and provide evidence for early prevention of sleep disorders in post-ICU patients. Methods PubMed,Embase,Web of Science,the Cochrane Library,CINAHL,CBM,CNKI,WanFang Data and VIP were searched from inception to June 2022. There were 2 researchers who independently screened the literature and assessed the quality of included studies using Critical Appraisal Tools published by Newcastle-Ottawa Quality Scale(NOS). The meta-analysis was performed by StataSE.12 software. Results A total of 28 studies were included in this study with a total of 3 170 patients. The prevalence of subjective sleep disorders by random effects model was 51.84%[95%CI(45.17%~58.51%)]. The results of subgroup analysis showed that the prevalence of subjective sleep disorders within 1 month,1~3 months,>3~6 months and >6 months after ICU discharge was 50.51%[95%CI(40.97%~60.05%)]、53.34%[95%CI(44.86%~61.82%)]、49.44%[95%CI(32.71%~66.16%)]、29.55%[95%CI(13.28%~45.82%)],respectively. Meta regression results of single covariate showed that sample size and country were related to heterogeneity(P<0.05). Conclusion The prevalence of sleep disorders in post-ICU patients is high. Nursing staff should take relevant interventions,strengthen sleep health education and guidance for patients,and reduce the incidence.

Indicators to evaluate nursing staff allocation:a scoping review

Objective The scope of the literature related to nursing staff allocation was reviewed,so as to clarify the evaluation categories,connotations,applicable scenarios,application effects,evaluation tools,and contents of nursing staff allocation,as well as the differences between domestic and overseas evaluation indicators. Methods Guided by the methodological framework of the scope review,we systematically searched domestic and foreign databases and guideline libraries such as CNKI,Wanfang,PubMed,CINAHL,Web of Science,and International Guideline Collaboration Network. The search period was from February 14,2012 to February 14,2022. The included studies were summarized and analyzed. Results 371 papers were included. The indicators appeared 632 times in total,including 338 patient-related indicators,181 nurse-related indicators and 113 organization-related indicators. The top 10 indicators of occurrence frequency were equity,mortality,job satisfaction,quality of care and patient safety evaluation,patient satisfaction,hospital stay,infection,pressure injury,falls and bed fall,and the lack of nursing care. Foreign countries pay more attention to indicators such as infection,mortality,length of stay,readmission rate,and the lack of nursing care,while more emphasis were on fairness and satisfaction in our country. Conclusion At present,the construction of indicators underlying nursing staff allocation needs to be improved. It is necessary to standardize the names,connotations,evaluation methods and weights of indicators,and to build a set of indicator systems with strong guidance and high sensitivity.

Evidence summary for non-pharmacological interventions of breast cancer-related lymphedema

Objective To integrate the best evidence of non-pharmacological interventions for breast cancer-related lymphedema with evidence-based methods,and to provide evidence-based bases for standardized evaluation and management of lymphedema for clinical medical staff. Methods The best evidence about the prevention and management of lymphedema in breast cancer patients were searched systematically,such as relevant guidelines,expert consensuses,systematic reviews and so on. The retrieval period was from January 2016 to January 2022. Results A total of 25 pieces of the literature were included,including 6 guidelines,4 expert consensuses and 15 systematic evaluations,and 33 pieces of best evidences were integrated from 2 aspects,namely the preventive intervention and therapeutic intervention. Conclusion The summarized evidence can be used for the prevention and treatment of breast cancer lymphedema,and medical staff should provide specific health guidance for patients with lymphedema based on cultural background,clinical situation,and patients’ opinions.

Objective To construct a structural equation model to analyze the effect of various factors on the quality of life in stroke patients. Methods In this cross-sectional study, a total of 657 stroke patients from 2 tertiary A hospitals in Shanghai were enrolled as investigation subjects via convenience sampling methodology. General information questionnaire,Sense of Coherence Scale-13,Perceived Social Support Scale,Medical Coping Modes Questionnaire,National Institutes of Health Stroke Scale,Self-Esteem Scale and Stroke-Specific Quality of Life were utilized as the investigating tools;Mplus 7.4 was used to construct a structural equation model. Results The average score of quality of life in stroke patients was(178.30±30.30);neurological status,self-esteem were negatively cor-related with quality of life(P<0.05);sense of coherence,social support,coping style were positively correlated with quality of life(P<0.05);sense of coherence,taking a confrontational approach for coping,yielding and neurological status had a direct impact on quality of life(path coefficients were 0.471,0.169、0.135,-0.089,P<0.05);social sup-port,self-esteem,sense of coherence and neurological status had an indirect impact on quality of life(path coefficients were 0.232,0.108,0.058,-0.058,P<0.001). Conclusion The quality of life in stroke patients is influenced by multi- ple factors interactively,and sense of coherence is an important factor affecting the quality of life;adopting a confron-tational approach as coping styles,good social support and high self-esteem can improve the level of quality of life.

Objective The scope of literature review correlated to narrative intervention for patients living with breast cancer was conducted to define the object,form,content,timing/duration and patient outcomes. It can provide references for future research and development of efficient,scientific,and rigorous narrative intervention programs for breast cancer patients and guide clinical practice. Methods According to the scoping review methodology,we systematically searched PubMed,CINAHL,Embase,Web of Science,Cochrane library,CNKI,WanFang database and VIP database. The search period is from the establishment of the database to December 31,2022. There are 2 researchers who analyzed the included studies independently. Results A total of 25 papers from 6 countries were included,and 12 of which coming from China. The results found that the narrative intervention runs throughout the treatment of breast cancer patients. Patients at different treatment stages have different needs for narrative intervention. Less attention had been paid to breast cancer survivors. The forms of narrative intervention include “telling story”,“writing story” and “watching/reading story”. Most of narrative practice forms were “telling story”. Overall,the narrative intervention had significant improvements in psychological,physical,cognitive and life quality,family relationship/social support for breast cancer patients. Conclusion Narrative intervention has positive effects on patients with breast cancer. Currently,the forms of narrative practice for patients with breast cancer need to be further enriched. More attention should be paid to breast cancer survivors,and narrative intervention programs should be individualized according to patient characteristics to enhance intervention effects.

Objective To construct a lifestyle management scheme for infertile women with polycystic ovary syndrome(PCOS) during outpatient medical treatment,and to evaluate its clinical effects. Methods Based on the evidence summary and qualitative interviews,a lifestyle management scheme for infertile women with PCOS during medical treatment was constructed. The infertile women at childbearing age with PCOS from the health management center and reproductive medicine center of a tertiary A hospital in Nanjing were recruited through convenience sampling. 27 patients who visited the above mentioned 2 centers from September 15 to 30,2021 were recruited in an experimental group successively,while another 27 patients who visited there from February 15 to 28,2021 were recruited in a control group successively. Next,the lifestyle management scheme was implemented in the experimental group in 9 key steps,including evaluation,screening,status analysis,formulation of body mass management objectives,diet management,exercise management,behavior management,health guidance,and evaluation. Meanwhile,27 patients in the control group were given the routine healthy lifestyle guidance. After 3 months,the figure of BMI,insulin resistance index,serum testosterone,anxiety,depression and quality of life of the 2 groups were analysed. Results After the intervention,the BMI of the experimental group was 24.43±2.99,which was lower than (26.48±3.49) in the control group. The insulin resistance index was 2.42±0.98,which was lower than 3.52±0.97 in the control group. The level of testosterone was(0.55±0.19) ng/ml,which was lower than(1.20±0.42) ng/ml in the control group. The anxiety level was(5.73±2.13) and the depression level was(5.12±2.66),which were lower than(9.40±2.36) and(10.60±2.58) of the control group. The score of disease-related quality of life was(140.38±7.25),which was higher than(115.56±7.99) of the control group,and the differences were statistically significant (P<0.05). Conclusion The implementation of the lifestyle management scheme for infertile women with PCOS during medical treatment is conducive to adjust patients’ endocrine and metabolic status,alleviate their anxiety and depression and improve quality of life.

Objective To develop an evidence-based decision support system for nutritional risk screening and assessment in children with congenital heart disease(CHD),and to evaluate its clinical effect. Methods The best evidence for nutritional risk screening and assessment in CHD children was summarized,and an evidence-based decision support system was developed. A before-and-after study design was adopted,and the medical staff and children with CHD admitted to the Cardiac Intensive Care Unit of a tertiary A-level hospital for children in Shanghai were selected as the research subjects. The data of the experimental group was collected from September to December 2021,while the data of the control group was collected from September to December 2020. The outcome measurement were the consistency of nutritional risk screening results of the responsible nurses and nutrition specialist nurses,the time-consuming of nutritional risk screening,the proportion of children receiving nutritional assessment and intervention,the response time of high-risk warning value of nutritional screening,and the satisfaction of medical staff with the use of information system. Results 191 children were included in the experimental group,and 193 were in the control group. The consistency coefficient of the nutritional risk screening results was 0.890 in the experimental group,and it was 0.689 in the control group. The nutritional risk screening time was(2.97±1.05) min in the experimental group,and it was(4.05±1.01) min in the control group. The proportion of infants who received nutritional assessment in the experimental group was 95.29%,compared with 89.64% in the control group;the proportion of infants receiving nutritional intervention was 69.11% in the experimental group,and 54.92% in the control group. The response time of the high-risk warning value of nutritional screening in the experimental group was(4.73±2.31) min,while the control group was(41.53±9.13) min. The satisfaction score of medical staff on the use of the clinical nursing information system was(108.04±11.31) points,and it was(92.89±9.37) points in the control group. The above data were compared between the 2 groups,and all the difference were statistically significant(P<0.05). Conclusion Using the evidence-based decision support system for nutritional risk screening and assessment of CHD children can improve the consistency of nutritional risk screening results,shorten the time-consuming of nutritional risk screening,promote nutritional assessment and nutritional intervention by medical staff,and improve satisfaction with the use of information system.

The study introduces the hazards of fatigue in patients with interstitial lung disease,and describes the common measurement tools for fatigue symptoms in ILD. Management measures to improve their symptoms are summarized in order to provide a basis for the development of comprehensive,multidisciplinary and individualized ILD fatigue management programs in the clinical settings.

Objective To identify the specific interventions,content elements and application effects of telehealth,so as to provide the guidance for future research. Methods The literature was retrieved from PubMed,Embase,Cochrane Library,Web of Science,CNKI,Wanfang,and CBM databases. The included studies were summarized and analyzed. The search time was from the establishment of the databases to January 5th,2023. Results A total of 29 articles were included. The intervention forms included application,portable electronic device,network communication software,and website. The content elements included remote monitoring,education,rehabilitation,outpatient,psychological and cognitive intervention. The outcome types were mainly physiological indicators,psychological indicators,self-management,clinically outcome,and feasibility. Conclusion The main forms of continuous nursing are mobile applications and portable electronic devices. Comprehensive content and rich functions of devices are more favored by patients. Telehealth intervention is feasible,safe,and effective. Medical workers can further broaden the service of continuous nursing for patients with lung transplantation by remote technology. In the future,studies on telehealth intervention are still needed to promote the health of patients and improve the outcomes of patients.

The impact of discharge planning demand score for length of stay in older patients with chronic heart failure

Objective To evaluate the value of discharge planning demand score for prediction of prolonged length of stay in older patients with chronic heart failure,and analyze corresponding nursing strategies. Methods From November 2021 to June 2022,343 older patients with chronic heart failure admitted to the cardiology wards of a tertiary hospital in Beijing were recruited,and the general data,the score of discharge planning demand and the length of stay were collected. We divided the patients into 2 groups according to the length of stay. Binary logistic regression analysis was used to explore the influencing factors of prolonged length of stay. Predictive value of discharge planning demand score for prolonged length of stay was assessed by receiver operating curve(ROC) analysis. Results There were 343 older patients with chronic heart failure included in this study. Length of stay was prolonged in 136 cases(>7 d). The results of binary logistic regression analysis showed that living alone(OR=2.721,P=0.027) and the discharge planning demand score(OR=1.503,P<0.001) were independent predictors for pro-longed length of stay. The area under the ROC curve was 0.828(95%CI 0.781~0.876,P<0.001);the optimal critical value was 3.5,and the corresponding sensitivity was 70.6% and the specificity was 85.5%. Conclusion The discharge planning demand score has good predictive value for the prolonged length of stay in older patients with chronic heart failure,so as to be used for early identification of high-risk individuals with prolonged length of stay.

Objective We systematically retrieved and evaluated risk prediction models for liver failure after hepatic resection,so as to provide a reference for constructing higher quality risk prediction models for liver failure after hepatic resection. Methods CNKI,Wanfang,sinomed,PubMed,web of science,Embase,Cochrane Library,CINAHL databases were searched for studies on the construction of risk prediction models for liver failure after hepatectomy. The literature was independently screened by 2 researchers and the information was extracted,and the risk of bias and applicability of the included literature was evaluated according to the Predictive Modelling Study Data Extraction Form and the Risk of Bias Assessment Tool. Results A total of 18 studies on the construction of risk prediction models for liver failure after hepatectomy were included,involving 21 models with an area under the subject working characteristic curve of 0.690~0.883. The independent predictors that were repeatedly reported by the models were total bilirubin,residual liver volume,platelet count,prothrombin time,albumin concentration,liver stiffness,and extent of liver resection. Some predictors are controversial,including male,laparoscopic surgery,liver-muscle ratio. The risk of bias in all studies is high,mainly due to insufficient sample size,poor treatment of continuous variables,lack of model performance assessment and over-fitting of models. Conclusion The existing models for predicting the risk of liver failure after hepatectomy are still in the development stage,and there are certain commonalities and controversies in the predictors,which can be further improved by conducting model validation studies or developing localized prediction models with good performance in the future.

Effects of non-nutritive breast sucking on the oral feeding ability of very low birth weight preterm infants

Objective To investigate the effect of non-nutritive breast sucking on oral feeding ability in very low birth weight preterm infants. Methods From July 2020 to June 2021,148 very low birth weight preterm infants admitted to the NICU of a tertiary children’s hospital in Shanghai were consecutively included,and they were divided into an experimental group and a control group with 74 cases in each group by block randomization. After the mechanical ventilation weaned,infants in the experimental group received non-nutritive breast sucking once a day for 5 minutes each time,and sucked the pacifier for 5 minutes before each tube feeding. The infants in the control group only sucked the pacifier for 5 minutes before each tube feeding. The time to achieve full oral feeding,the corrected gestational age at full oral feeding,the score of the Breastfeeding Behaviour Scale for premature infants,the days of breast feeding,and the breast feeding rate on the day of discharge were compared between 2 groups. Results There were 2 cases dropped in the experimental group and no cases dropped in the control group. The time to achieve full oral feeding was(12.29±4.12) d in the experimental group and(16.87±7.42) d in the control group. The difference between 2 groups was statistically significant(P<0.001). The corrected gestational age at the time of full oral feeding was(36.45±1.54) weeks in the experimental group and(36.13±4.59) weeks in the control group,and there was no significant difference between 2 groups(P=0.574). At the time of discharge,the Breastfeeding Behavior Scale score was[12.0(10.0,13.0)] in the experimental group and[6.0(1.5,8.5)] in the control group. The difference between 2 groups was statistically significant(P<0.001). During hospitalization,the days of breastfeeding in the experimental group were[47.0(33.8,62.8)] d,while those in the control group were[37.5(19.0,56.3)] d. The difference between 2 groups was statistically significant(P=0.021). The breastfeeding rate on the day of discharge was 84.7% in the experimental group and 66.2% in the control group,and the difference between 2 groups was statistically significant(P=0.012). Conclusion The implementation of non-nutritive breast sucking can shorten the time to achieve full oral feeding in very low birth weight preterm infants,promote their breastfeeding behaviour,increase the days of breast feeding with their own mother’s milk during hospital stay,and improve breast feeding rate.

Methods We searched BMJ Best Practice,UpToDate,Joanna Briggs Institute,Guidelines International Network,Agency for Healthcare Research and Quality,Canadian Medical Association CPG Infobase,Cochrane Library,Yimaitong,Extracorporeal Life Support Organization Website,PubMed,Web of Science,CINAHL,Embase,MEDLINE,CNKI,Wanfang database and VIP database on all evidence related to the management of inter-hospital transport in patients on ECMO,including clinical decisions,guidelines,evidence summaries,best practices,expert consensuses,systematic reviews and meta-analysis. The retrieval time limit is from the establishment to September 2022. There were 4 investigators who independently assessed the quality of the literature,and also extracted,integrated and graded the evidence from the included literature. Results Totally 13 articles were included,involving 2 guidelines,8 expert consensuses and 3 systematic reviews. Finally,37 pieces of best evidence were summarized from 4 dimensions,namely team composition,response and preparation,transportation practice,management and quality control. Conclusion This study summarized the best evidence which can provide a basis for the implementation of inter-hospital transport supported by ECMO to promote patient safety during inter-hospital transport.

Objective To explore the influencing factors of the transition to adults on children with cancer from the perspective of professional care providers,so as to improve their transition readiness. Methods From September 2020 to June 2021,by phenomenological research methods,semi-structured in-depth interviews were conducted with 5 doctors,19 nurses and 3 social workers. The interview data were transcribed,coded,analyzed and refined. Results From the perspective of professional care providers,the factors influencing the transition of children with cancer can be summarized into 4 factors,i.e. individual,family,medical system and society,and 9 sub-factors,i.e. diagnosis and treatment experience,disease cognition,autonomy,transitional consciousness,education style,professional team,profes-sional service,social environment and service system. Conclusion There are multiple factors influencing the transition to adulthood. To assist them to effectively cope with the long-term impact of disease and treatment,it is necessary to raise the public attention,encourage children to be informed and participate in disease management,upgrade professionals’ capabilities,strengthen the continuity of long-term follow-up,develop the service system,and develop individualized and targeted intervention adapted to Chinese situations.

A research on the characteristics of cardiovascular health score trajectories in patients after percutaneous coronary intervention

Objective To investigate the characteristics of cardiovascular health score trajectories in patients after percutaneous coronary intervention(PCI). Methods The convenience sampling method was used to recruit 339 acute myocardial infarction patients who underwent PCI for the first time in the cardiology department of a first-class tertiary hospital in Harbin from May to October 2019 with regular follow-up after PCI. We used the group-based trajectory model to identify the heterogeneous trajectory groups of cardiovascular health score based on the baseline,6 months and 12 months follow-up. The Cox proportional risk regression model was used to analyze the effect of trajectory groups and major adverse cardiac events(MACE). Results 5 cardiovascular health score trajectories were identified as low-increasing group(19.8%),medium-declining and then increasing group(24.8%),medium-rapid increasing group(14.4%),medium-stable group(31.9%),high-stable group(9.1%). Cox proportional risk regression model showed that,after adjustment for potential confounding factors(age,sex,high-density lipoprotein cholesterol and alcohol consumption),the risk of MACE in medium-declining first and then increasing group,medium-rapid increasing group,medium-stable group and high-stable group were 0.498(P=0.033),0.446(P=0.049),0.403(P=0.006) and 0.259(P=0.030) times of low-increasing group,respectively. Conclusion Acute myocardial infarction patients after PCI for a year showed various cardiovascular health score trajectories;the improvement and maintainance of a high level of cardiovascular health score were negatively correlated with the risk of MACE. Medical staff can monitor the characteristics of cardiovascular health score changes dynamically,strengthen risk factor management and adopt early intervention methods to improve prognosis.

Objective To systematically evaluate the experience of elderly patients on coping with multiple chronic conditions(MCCs).Methods Pubmed,the Cochrane Library,Web of Science,EmBase,CNKI,Wanfang Database and VIP database were searched for qualitative research studies on experience of elderly patients with MCCs. The retrieval period was from inception to August 2021. The Joanna Briggs Institute Critical Appraisal Tool for qualitative research(2016) in Australia was used to evaluate quality of the studies. A meta-synthesis method was used to integrate the results.Results A total of 15 studies were included;60 findings were extracted;10 cate-gories were summarized;3 integrated results were formed,namely changes caused by MCCs and aging,reflection and response to MCCs,adjusting to a normal life.Conclusion Aging with MCCs brings many challenges to the life of elderly patients. Health professionals should pay attention to the different coping styles of patients with MCCs,and develop feasible and support programs to help them better cope with MCCs which derived from their wisdom.

Objective To explore the true perception of patients with breast cancer who participated the rehabilitation exercise of limb function.Methods Using the purposive sampling method,18 postoperative breast cancer patients admitted in a tertiary hospital in Wuhan,China,from February 2021 to March 2021 were recruited for semi-structured interviews based on the theory of planned behavior. Collected data were analyzed to summarize the main themes by the Colaizzi phenomenological method(7 steps).Results The true experience of postoperative breast cancer patients on the rehabilitation behavior of limb function can be summarized as 3 themes. Behavioral attitude:negative emotions following stress,rehabilitation behaviors that are systematically underestimated,and the value of reconstructing the image of women. Subjective normative measurement:influence of social roles,peer effect,and medical staff support. Perceived behavior control:the confinement of the comfort zone,and the needs of the rehabilitation environment.Conclusion The behavior of limb function rehabilitation in postoperative breast cancer patients is affected by the behavioral attitude,subjective norms measurement and perceived behavioral control. From the perspective of patients,medical staff should correct cognitive bias in rehabilitation behavior of breast cancer patients. Negative cognition needs to be avoided through positive belief factors. Meanwhile,multiple sources of support should be highlighted to stimulate patients’ sense of self-management,create a rich rehabilitation environment,and promote the improvement of breast function rehabilitation behavior of postoperative breast cancer patients.

The experiences of patients with myocardial infarction returning to society after being discharged from the hospital:a meta synthesis of qualitative studies

Objective The aim of this study was to perform a meta-synthesis of findings of myocardial infarction(MI)patients’ experiences of recovery after returning to society. Methods We searched databases including the Cochrane Library,Joanna Briggs Institute Library,PubMed,Embase,Web of Science,PsycINFO,CINAHL,CBM,WanFang,CNKI and VIP. All of the qualitative research on the real experience of MI patients returning to society after being discharged from the hospital were collected. The search time limit is from the establishment of the databases to November 2021. The JBI critical appraisal tool for qualitative research in Australia was used to evaluate the quality of the included studies,and the results were summarized and integrated by integrating methods. Results A total of 13 original studies were included. 47 findings were collected,and then the similar results were grouped into 11 new categories,and finally these categories were integrated into 3 synthesis results. Integration result 1:living with psychological loads,and the patients were facing physical and mental challenges. Integration result 2:reflecting on the value of life and strive to change one selves. Integration result 3:recognition of the world,and desire for outside help. Conclusion Although MI patients have been treated to save lives,they are still living under different degrees of pressure after discharging from the hospital. The future life is full of uncertainty,and each person’s coping ability is different. Health care workers and members of the public society need to pay more attention to their feelings and needs after returning to society,and to provide necessary health care knowledge and living skills to help them learn to coexist with the disease and better adapt to social lives.
